What is Coordinated Family Support:

 

CFS is a new service option specifically designed for adults who are 18 years and older, with a Developmental disability and who:

  • Choose to live in their family home

  • Are served by a regional center

CFS provides assistance with the coordination of services and supports that allow adults to continue living in their family home such as:

  • Developing skills in the home

  • Coordinating & getting services delivered, including help with “generic services.”

    • Generic Resources

      • Cal-Fresh

      • IHSS

      • SSI

  • Helping people access their community.

    • Social Rec Programs

    • Social Programs

    • Community Integration

    • Access to educational support

    • Therapy

    • Access to medical services

    • ETC

  • Conservatorship

  • Coordination of services

CFS providers will prepare a service plan designed to help reduce or remove barriers to staying in the family home, address challenges and meet goals.  Services will be provided in a manner that respects the language & culture of individuals and their families, and services will be tailored, individualized, and flexible to meet the changing needs and preferences of individuals.

 

Who is eligible for Coordinated Family Support?

 

  • CFS is for adults (18 years and older)

  • live with their family

  • receiving services from a Regional Center.

 

Services/Support offered through Coordinated Family Support:

 

CFS is a service that will be tailored, individualized and flexible to meet the changing needs and preferences of the individual being served along with their family/caregiver.

 

 

How can I Sign up for Coordinated Family support Services?

 

CFS Process

  • Notify your regional Center Service Coordinator

  • They will schedule at IPP team meeting to determine how CFS can assist.

  • Referral packet is then sent to the CFS provider for review.

  • CFS provider will reach out to you to schedule an meeting.

  • You, your family and the CFS provider will then discuss a plan to identify services needed.

  • Plan of action is developed.

  • After the plan is developed and approved, services are started.
